Rocky Knoll Health Care Center is licensed and certified as a Skilled Nursing Facility (SNF) offering short-term rehabilitation and long-term care. At Rocky Knoll, we provide high-quality, skilled nursing and health care services to the residents of Sheboygan County. We maintain a proactive approach in anticipating the individual needs of each resident, ensuring that their health and welfare requirements are met.

Our caregivers include Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants. Rocky Knoll's History in Care
We have a rich history of providing care for Sheboygan County. Through the years, the emphasis of people in our care has changed, but our foundation has and always will be “Innovation with Compassion.” As health care continues to evolve, we will lead the way in new areas of service and care.

August 1926
Rocky Knoll Health Care Center began in August 1926 as a facility to care for Sheboygan County residents diagnosed with tuberculosis.
1950’s
By the 1950’s, advances in medical treatments eliminated the need to have a facility dedicated to that type of care so began focusing on caring for seniors. Soon after the passage of Medicare/Medicaid legislation in 1965, we began to provide services to Medicare-eligible guests.

Additions Starting in 1972
There have been three significant additions to our facility: in 1972 the two story west building was constructed, in 1992 the north building, and, in 2002 which is now dedicated to short term rehab.

2022 Present
Today we are licensed to serve up to 149 residents, providing short-term rehabilitation and long-term care that meet residents' individual needs.
