OptumCare
A UnitedHealthcare Nursing Home Plan is a Medicare Advantage plan and may be a good choice for health care coverage. With this plan, you receive more benefits than with Original Medicare, including Part D prescription drug coverage. Members also receive the support of a dedicated OptumCare® advanced practice clinician. The OptumCare clinician provides care coordination and is dedicated to your unique needs and concerns.

Plans are insured through UnitedHealthcare Insurance Company or one of its affiliated companies, a Medicare Advantage organization with a Medicare contract. Enrollment in the plan depends on the plan’s contract renewal with Medicare. These plans are available to anyone living in a contracted nursing home and requires an institutional level of care. Advocate Aurora Post Acute Network Program
To support the goal of providing high-quality post-acute care, Advocate Aurora has developed a preferred network of community-based skilled nursing, long-term acute care, and rehabilitation providers who meet Advocate’s standards for quality. This Post-Acute Network (PAN) includes 41 skilled nursing facilities (SNFs) that have partnered with Advocate to provide optimal care through its SNF Care Program. Rocky Knoll is proud to be a member of this network.
In this care model, SNF patients are seen by an on-site provider—an Advocate employed advanced practice nurse (APN) or physician— three to five days per week. The APN helps ensure a smooth transition, communicates with the SNF physician, is closely involved with the SNF staff to facilitate medical care coordination and discharge planning, and is responsible for relaying medical information back to the referring PCP.
Hospice Care
Through a coordinated effort with local professionals trained to address all concerns involved in Hospice care, our team coordinates your care with your Hospice Provider. The team consists of nurses, chaplain, social worker, nurse’s aides, and volunteers to assure your physical, spiritual, emotional, and social well being.
